Release of Information

Affidavit of Enrollment (DR 2643)

A Turning Point follows state and federal rules and guidelines regarding the release of client information. In order for A Turning Point to release any records to anyone, other than the client, A Turning Point must have a valid release of information on file.

Affidavit of Enrollment (DR 2643)

Need to provide proof of enrollment and attendance in Level II DUI/DWAI treatment to the Colorado Department of Revenue to get your license; complete the Affidavit of Enrollment (DR 2643) below.

Once completed, our office will send a finalized copy of the Affidavit of Enrollment to the Colorado Department of Revenue and to the client.

The Colorado Behavioral Health Administration (BHA) collects and stores all Level II DUI/DWAI treatment records. If you are in need of documentation of any attended or completed Level II DUI/DWAI treatment, complete the DRS Request Form below.

If you are needing to request client treatment records for yourself or for a client, complete the Records Request Form below. Once received, and a Release of Information is verified to be on file, the requested records will be provided within 48-72 hours.

Please attach any Release of Information and applicable Billing and Invoicing information.
